Lakeview Clinic been approved as a registered vaccine administration site. We are receiving a supply of vaccine and are scheduling appointments for Lakeview Clinic patients that are 5-11 years old for Pfizer Pediatric Vaccine and 12 years of age or older for Pfizer Adult Vaccine. We currently only have Pfizer in our vaccine inventory. Pfizer requires two shots with the second dose given 21 days following the first dose.
Lakeview Clinic is accepting phone call scheduling for COVID-19 vaccine appointments for 5-11 year olds on clinic dates outlined below. Lakeview Clinic is performing online scheduling for 12+ for appointment dates listed below. Please ask your nurse or provider about the COVID-19 vaccine during your appointment with us!
Lakeview Clinic is currently requiring that all 3rd doses of the COVID-19 vaccine be approved by your primary provider. Your provider will evaluate if you meet the following conditions to receive a booster injection for COVID-19. Third doses are administered a minimum of 28 days after you had received your second dose of either Pfizer or Moderna. Lakeview Clinic only is stocking Pfizer at this time.

Lakeview Clinic is currently requiring that all 4th doses of the COVID019 vaccine be approved by your primary provider. Your provider will evaluate if you meet the following conditions to receive a 4th dose (2nd booster) injection for COVID-19. In order to qualify you must:

Lakeview Clinic is also administering booster doses of Pfizer vaccine to patients that are eligible. In order to qualify to receive a booster dose of the Pfizer vaccine, patients must fall into one of the following categories and have received their 2nd dose OF PFIZER OR MODERNA 5 months or longer ago, OR JOHNSON AND JOHNSON 2 or more months ago:
As mentioned, Lakeview is only stocking Pfizer at this time. With CDC’s new recommendations, you may choose to receive Pfizer as your booster even if your vaccine series was completed with Moderna or Johnson and Johnson. If you would like to stick with the same manufacturer you had initially received, please check with your local pharmacy or Carver County Public Health to receive the other vaccines.

Lakeview Clinic has now begun to administer the Pfizer 5-11 vaccine to those patients in this age group. We are hosting clinics that are dedicated to serving this population.
The 5-11 Pfizer vaccine is administered to patients in the same fashion the adult vaccine is administered. Patients may call our front office at 952-442-4461 to schedule their appointment on the days that are outlined. This vaccine is a two dose series, with the second dose being administered twenty-one (21) days after receiving the first dose. With your second dose being directly tied to the first, please make sure that your child is available to receive the second dose on the specific day assigned to them. Our front office will automatically schedule your second dose appointment to minimize the possibility of a missed dose.

Below are the schedules for the Pfizer vaccine. Adult schedules are for 12 years old and above, and pediatric schedules are for ages 5-11. Adult COVID-19 Schedules are all facilitated online. If there are appointment slots available, the date will be clickable for online scheduling. If the clinic is full for the day, the schedule date will not allow you to proceed to schedule. Please call the clinic to schedule your dose of the 5-11 year old Pfizer vaccine. If there are no clickable dates outlined in the Adult COVID-19 Schedules, the clinics are full.
