Appointments
Appointments are available during clinic hours. When making your appointment, please:
- Be specific about the reason for your visit, so we may schedule adequate time
- Have your insurance information available
Please bring all current medications with you to the clinic at the time of your appointment. If you are unable to keep an appointment, please notify the clinic as soon as possible. For appointments scheduled more than two days in advance, we will send out a text reminder of your appointment.
Phone Calls
Urgent medical questions will be transferred to trained nurse triage staff who will answer your questions or further direct you. Routine questions may be answered by a return call from a physician or nurse. Please call back if your call is not returned in a reasonable amount of time.
Emergency/After Hours
For life threatening problems, dial 911. Physicians are available for other urgent concerns and may be contacted through the answering service by dialing 952-442-4461.
Patient Tools
Registration
Please check in at the reception desk each time you visit the clinic. Bring all relative insurance identification cards with you. Co-pays are due at the time of your visit. Failure to identify your insurance could result in out-of-pocket expenses for you. After registering, you will be directed to the appropriate location within the clinic for your visit.
Test Results
Test results may be available at the time of your visit. If not, results are typically provided within one week. If you do not hear from the clinic in one week, please call us and ask for your test results. Do not assume test results are normal if you have not been notified.
Medical Records
Your medical records are strictly confidential and can not be disclosed to anyone without your written permission. You must sign a special written consent form if you wish to authorize the release of this confidential information from Lakeview Clinic.

Patient Portal
The Patient Portal provides access to your medical information online. Please click on the Resource Guide below to learn more about the Patient Portal or for some helpful troubleshooting tips.
Prescription Refills
For prescription refills please call your pharmacy.
Online Bill Pay
Lakeview Clinic now offers the ability to pay your bill online. You can access this feature using the link below. You will also need some information from your patient statement. You will be able to create an account to allow for easier future payments. You are also able to access this feature from within your Patient Portal account if you have signed up for that service.
